Records library
One workspace for every governed record — open, closed, challenged, rolled back, forked and superseded. Full-text search, faceted filters, saved views and sorting. Authorized users can open old records indefinitely, subject to retention and legal hold. There is no ordinary hard-delete.
Evidence workbench
Atomic claims with epistemic state, source spans, freshness, identity state and trust scope. Contradictions are shown as contradictions. Suggested links always require explicit human adoption.
Harm gate and door assessment
Per-harm impact and likelihood produce a derived gate. Separately, the door axis records whether there is a real door, a wall, a gap or an UNKNOWN, with a minimum grade — and an achieved grade only where a concrete door exists. Three axes, three readings, no composite score.
Effectiveness verification
A sealed plan with metric, baseline, threshold, sample, window and maturity date, plus a verifier who is structurally distinct from the repair owner. Changing the criteria creates a successor plan; it does not quietly move the goalposts.
Lineage and relationships
Forks from snapshots, successors and secondary CAPAs are recorded as append-only relations with forward and reverse labels. Event streams are never merged, and a child never changes its parent.
Historical review
Two separate timelines — content history and canonical lifecycle plus receipts — with as-of viewing, immutable prior versions, field and evidence diffs, receipt-chain verification and export of a review package.
Cycle Guide
A navigator, not an agent. It explains the phase and exact state, the permitted next work, which guards are satisfied, failed or unknown, and what is missing. It cannot submit, approve, sign, transition or resolve an UNKNOWN.
Installable and responsive
One responsive application, installable on desktop, tablet and mobile. Online-first by design: no offline state transitions, approvals, signatures or receipt issuance.
